Losing weight with a controlled diet and regulat exercise should definitely be tried. Some drugs (affecting fat metabolism) may support this process. No medication should be used unless it has been approved by an endocrinologist.
Morbid obese people who cannot lose weight after at least 2 diet programmes or a diet done at least for 6 months or the ones putting on weight again after losing some are candidates for obesity surgery.
